I can't really help, other than mention that I have the same problem with my SB Live card.

Sorry for going off topic here, but I'm curious how you got quake 4 running. On my machine, after installing from the port and running 'quake4', I get a wavy screen (like when you select a resolution that your monitor doesn't support in Windows). Best case scenario I have to -KILL quake4, worst case I have to ctrl+alt+backspace to kill the X server. Manually running the linux installer results in a locked system.

Yes it is off topic :)

I had same problem too, i think. You can check yours GL librarys.

Here:

$ pkg_info -L nvidia-driver-1.0.8178 | grep libGL
/usr/X11R6/lib/libGL.so.1
/usr/X11R6/lib/libGLcore.so.1
/compat/linux/usr/lib/libGL.so.1.0.8178
/compat/linux/usr/lib/libGLcore.so.1.0.8178
$
$ ls -l /compat/linux/usr/lib/libGL*
lrwxr-xr-x 1 root wheel 32 Jan 12 13:47 /compat/linux/usr/lib/libGL.so -> ../../usr/X11R6/lib/libGL.so.1.2
lrwxr-xr-x 1 root wheel 17 Jan 1 16:14 /compat/linux/usr/lib/libGL.so.1 -> libGL.so.1.0.8178
-r--r--r-- 1 root wheel 535264 Jan 1 16:14 /compat/linux/usr/lib/libGL.so.1.0.8178
lrwxr-xr-x 1 root wheel 33 Jan 12 13:47 /compat/linux/usr/lib/libGLU.so -> ../../usr/X11R6/lib/libGLU.so.1.3
lrwxr-xr-x 1 root wheel 21 Jan 1 16:14 /compat/linux/usr/lib/libGLcore.so.1 -> libGLcore.so.1.0.8178
-r--r--r-- 1 root wheel 8136248 Jan 1 16:14 /compat/linux/usr/lib/libGLcore.so.1.0.8178

And you can test without "fullscreen":

$ grep r_fullscreen .quake4/q4base/Quake4Config.cfg
seta r_fullscreen "0"
$
